Overview

The Associate Professional (AP) will work under the supervision of the QIDP in delivery of consumer support services for assigned location. This position works with personnel to achieve service, clinical, and financial objectives within general resource and reimbursement models. In this role, you will ensure compliance with regulatory requirements through quality assurance activities.

    Ensures consumer and guardian participation in development of service plan and personal futures plan
  • Coordinates development of each person(s) served personal futures plan and coordinates scheduling of team meetings (times, dates, locations, etc.) and informs all team members of such
  • Schedules and facilitate screening for program vacancies and develops waiting list for potential consumer vacancies
  • Under supervision of QIDP, develop and implement service plan within 30 days of screening and admission for services, annually, and/or when significant changes occur
  • Monitors to ensure all service sites deliver services in accordance with contractual, legal and regulation requirements and implements/coordinates any necessary plans of correction from a regulatory body
  • Coordinates with QIDP to implement and monitor any necessary plans of correction from a regulatory body
  • Monitors all incident reports to ensure patterns of incidents are addressed and shares all incident reports with appropriate external agencies (area programs, social services, etc.) when necessary and appropriate
  • Immediately reporting any incidents of consumer abuse, neglect, or mistreatment to QP, BM, ED to ensure timely reporting to all applicable internal and external bodies so investigations are timely and thorough. Plan of Correction is initiated to prevent recurrences
  • Under supervision of QIDP, monitors/implements delivery of service plans and personal futures plan and training of staff
  • Ensures all written training programs are implemented and revised as needed
  • Documents consumer progress or regression
  • Initiates discharge planning if appropriate. Maintains records appropriately
  • Implements/monitors compliance with All Ways Caring Health and Safety programs
  • Maintains and/or monitors training reports to ensure timely training of staff for all assigned service sites
  • Participation in annual and routine supervision plan with QIDP relevant to NC Administrative Code: 10A NCAC 27G .0203
Qualifications
  • Bachelor's degree in human service-related field or Licensed RN
  • Ability to communicate both written and verbally
    Working knowledge of Federal, State, and local regulations concerning services to individuals with intellectual and/or other developmental disabilities
  • Ability to drive between company locations and service sites
  • Moderate travel 25-75%

Salary Range
USD $17.00 / Hour
